What are levels of care?

Levels of care refer to how much assistance a person needs with health management, as well as activities of daily living. Activities of daily living are tasks like dressing, taking a bath or shower, using the restroom, and eating.

Additional considerations include whether the individual can take prescription medications or manage chronic conditions independently, or whether some assistance from a caregiver is needed.

Concordia at Cabot Contracts and Cost

Plan Types

  • I have had therapy for my knee as an outpatient. It was a great experience being treated with great care. The Physical Therapist's were thorough and very helpful. I would recommend the facility to anyone who needed help.

  • My husband was transferred to Concordia for rehab. The care he received was outstanding. He has other health issues which made his stay .. somewhat challenging. I can’t name everyone so I have to say excellent job from the cna’s to the nurses to the therapist. The food was delicious. TheMore

  • My father, an Alzheimer patient had an unfortunate fall that required skilled nursing care. He was admitted after his hospital stay and has progressed from the skilled living facility to the personal care facility. The nursing and Dr staff provided the care required so that Dad has now become functional again. HeMore
